Support at Home program manual – A guide for registered providers
We aim to provide documents in an accessible format. If you're having problems using a document with your accessibility tools, please contact us for help.
This manual assists registered providers to understand their responsibilities and the delivery of care under the Support at Home program. It includes how the Support at Home program operates, how to work with participants to develop a care plan and a budget, information on participant contributions and the short-term pathways for AT-HM, restorative care and end-of-life services.
To support sector readiness, an early version of the manual has been released. Information may be subject to change and an updated version of the manual will be published once the Rules legislation is finalised.
If you have questions or feedback about this manual, email SAH.Implementation@health.gov.au.
We will review this manual regularly.
